Moving to the same folder breaks link

If a document is moved into the folder it is already a
member of, the link is removed instead of just being
left alone.

When editting a document it doesn't display the folder
information in the " Attaching to folder #" area.

When going to edit an existing document if a file isn't
selected in the Document File field the document will
no longer be associate.

An example is 1264.

When adding a document if the status is set to draft it
will show up as released anyway.
