Replacement file: nr-mysql-setup.sql

The 'nr-mysql-setup.sql' file that was packaged with
v0.4.3-0.4.5 has an error in the last 'INSERT' line.
Replace your default file with this file to properly
set up the database.

Problems may not be apparent until a server fails for
the first time, then it will not write to
the 'alert_log' database.

Thanks to Mark Stewart (once again) for bringing this
to my attention.

Fixed input-page.php file

A bug has been discovered that was a result of trying
to clean up code from version 0.2. The bug involves
an entry that is made from the node input page (input-
page.php). A record is entered into the "objects"
table of the database for each entry, but another
record SHOULD have been entered into the "alert_log"
table at the same time.

In future versions, this will be fixed, but in the
meantime, copy this file to your <html> directory to
replace the existing input-page.php file.

The node-runner.sql file packaged with version 0.2 has
a few oversights. Replace the original with this file
to properly import the necessary tables.
